Prevention that is actually about food, not fear
Bali belly is traveller's diarrhoea: unfamiliar bacteria, usually from water or food handling. The water rules are in our tap water answer. On food, the reliable heuristic is turnover and heat: a packed warung cycling through fresh nasi campur at lunch is safer than a quiet buffet that has stood since morning. Peeled fruit over pre-cut, cooked over raw at street level, and go easy on sambal the first days - chilli irritation gets misdiagnosed as illness constantly.
The 48-hour fix
Hydration is the whole game: water plus Oralit rehydration sachets (a few thousand rupiah at any Indomaret or Circle K), coconut water or Pocari Sweat between (Unicare Clinic). Eat plain - rice, toast, banana, clear soup. Local charcoal tablets (Norit) are a beloved folk remedy and harmless, but medical guidance does not actually endorse charcoal for traveller's diarrhoea, so do not rely on it. Loperamide (Imodium) is for bus rides and flights, not a cure - it parks the problem. Skip alcohol, coffee and dairy until solid.
When it is not just Bali belly
See a doctor if you have a high fever, blood in stool, signs of real dehydration, or no improvement after 72 hours. Clinics in Canggu, Seminyak and Ubud handle this daily; many do home visits with IV rehydration for 700,000-1,500,000 IDR (worth every rupiah on day two of misery).
